Medicare Care Plans provide people with two or more chronic health conditions and persistent pain or injury (exceeding 6 months) access to a Medicare rebate for up to 5 visits across two or more allied health professions. These visits can be a great way to kick start your rehabilitation to better function and fitness!

A few examples of chronic health conditions are diabetes, obesity or chronic musculoskeletal conditions (over 6 months). If you think you might be able to access the Medicare Care Plan program, speak to your GP at your next doctor’s appointment.

Target Physio’s team of Physiotherapists and Exercise Physiologists accept Care Plan referrals. Please note we do not offer bulk billing, your appointment will be charged in full at the time and a rebate will be processed for you to your medicare nominated account.
