Our Physiotherapists and Exercise Physiologists are registered DVA service providers and are accepting DVA referrals!
For Gold Card and White Card holders, please obtain a D904 referral from your doctor before your initial appointment with your Physiotherapist and Exercise Physiologist. The conditions you’re approved for under DVA must be listed on this referral in order for us to process claims.
For treatment from both a Physiotherapist and an Exercise Physiologist at Target Physio, you will need two referrals from your doctor.
Gold Card TPI holders do not require a referral prior to booking an initial appointment.
From October 1st, 2019 referrals to Physiotherapists and Exercise Physiologists require renewal, by visiting your doctor for another referral, every 12 appointments or every 12 months – whichever comes first. You can read more about these changes with DVA here.

What can you expect when you see an Exercise Physiologist at Target Physio?
Our admin team will alert you when you are close to the conclusion of your referral cycle period to discuss with your Physiotherapist or Exercise Physiologist as to whether you require further treatment.
At the conclusion of your referral treatment cycle, your treating practitioner will write a report to your doctor either outlining your improvements during the cycle and the review of your goals to request another referral or a summary of treatment completion and goals achieved.
Depending on your approved conditions, DVA can cover group classes as well, not only one-on-one appointments. So you can enjoy the social aspect of exercising with others during your treatment.
